Various factors increase acid & gastric juice production.
Classical causes of ulcers
Smoking
Alchole
Fatty meals
spices
are of relatively minor importance in the development of peptic ulcers.
NSAID still important.

NSAID still important.
Secretion of mucus layer is stimulated by certain prostaglandins.
NSAIDs block the function of cyclooxygenase 1 (cox-1), which is essential for the production of prostaglandins.
Newer NSAIDs (celecoxib, rofecoxib) only inhibit cox-2.

Helicobacter Pylori

90% of duodenal ulcers and up to 80% of gastric ulcers associate with.
Brak mucosal barrier.
Physically brak.
Bacterial enzymes.

H. pylori infection is common in general population.
About 20 percent of people under 40 years infected.
10% over 60 years infected.

Most infected people do not develop ulcers.
In countries with high socio-economic standards infection is less common than in developing countries.
Nobel Prize in Physiology or Medicine goes to Barry Marshall and Robin Warren 2005.

Peptic Ulcer

Gatric & duodenal ulcers due to brakdown of protective barrier.

Common in lower duodenum.

One in 10 develops an ulcer at some time in his or her life.

Why not get peptic ulcer?
Feedback mechanisum -
Acid in duodenum decrease gastrin.
Acid in s.intestin increase secretin.

Causes
Increase acid & pepsin secretion.
Brak down of mucosal barrier.
